Additional Insured Request Requirements
All additionally insured requests must be submitted to our general mailbox with the subject line - Additionally, Insured Request for our office to review and get that request made to our insurance company. Keep in mind for all additionally insured requests we do ask that you let us know at the time of booking that you will be making this request, and understand there is a $200 processing fee for our office staff to process the request for you, in addition to the requirement that you are using our professional staff.
